• Photocopy of ID (in black and white, not color) 
• Evidence of Citizenship
• Photocopy Evidence of Citizenship (in black and white, not color) 
• A $130 Payment for adults or $100 for minors - Payable to U.S. Department of State (Personal/Cashier’s Check OR Money Order ONLY)
• A $35 Payment - Payable to City of Calabasas (Personal/Cashier’s Check OR Money Order ONLY)

Passport renewals can be done on your own 

• Passport photos will not be available at this facility.
  
  
• All applicants must appear in person including children. 
  
  
• Applications must be typed or printed in BLACK INK. 

• PAYMENT: Pay by check or money order only
  Two separate checks are required. One for the State Department Fee and one for the City of Calabasas execution fee of $35. Standards fees are set by the U.S. Department of State.
